Application Checklist  

This checklist form can be used by individual applicants.  Before you submit your application, go though this checklist to make sure that you haven’t forgotten anything in your application package.            

Your application should include:

• A completed Application Form: Please complete all section of the application form. Leave no blank fields .
• Official University Transcript: If you are applying for a PhD, please include both the Bachelors and Master’s transcripts. Photocopies are not acceptable.
• Photocopies of Certificates from Colleges and Universities.
• Reference Letters: Three completed reference letters. The reference must be written by someone who has supervised the applicant in academic work or professional employment related to the proposed field of study.
• A copy of admission letter where applicable
